Personalized Home Care Through
AFC & GAFC
The Right Care, Right at Home
At Wells Health Care Services, we understand that every individual’s needs are unique. That’s why we offer two tailored programs— Adult Foster Care (AFC) and Group Adult Foster Care (GAFC), designed to provide the right level of support while ensuring comfort, independence, and quality of life.
What We Offer
We provide expert assistance with MassHealth eligibility verification, applications, renewals, and appeals. Our team helps seniors, families, and individuals with disabilities understand their options, submit accurate documentation, and secure the benefits they qualify for — without delays or confusion.
Adult Foster Care (AFC):
Adult Foster Care (AFC) is a MassHealth-funded program that allows seniors and adults with disabilities to receive daily care and supervision from someone they know and trust — often a family member. Qualified caregivers may receive a monthly tax-free stipend, along with clinical oversight and ongoing support, while helping their loved one remain safely at home.
- Often a family member or friend
- Right in the comfort of their own home
Group Adult Foster Care (GAFC):
Group Adult Foster Care (GAFC) is a MassHealth-funded program that provides daily personal care assistance and case management to adults living in qualified housing settings, such as senior apartments or subsidized housing. The program supports individuals who need help with activities like bathing, dressing, and medication reminders — allowing them to remain independent and avoid nursing home placement. It helps individuals remain independent at home instead of going to a nursing home.
